Welcome to the 2011-2012 Healthy School Champions' Score Card! Your school results will be confidential and only used by the Review Committee for recognition and planning purposes.

• New to the Healthy School Champions' Score Card?
  Begin by identifying a point of contact for your school. Each time you use the Score Card, you will be able to retrieve your school's information by using the point of contact's e-mail address.
• Returning to the Healthy School Champions' Score Card?
  If you completed the Score Card in the last school year (2010-2011), your results will be available once you login to the 2011-2012 site.
